2017 Service Management In Action Nomination Form

This “Service Management In Action” award is presented to one (1) person or organization that demonstrably achieved and/or delivered relevant or significant Business Outcomes utilizing Service Management processes, tools, governance, other industry frameworks, or Organizational Change Management (OCM) in the adoption of service management best practices within their organization for the previous yearly cycle.

This year’s award will be for SM projects/initiatives completed between 07/01/2016 – 06/30/2017.

Finalist Information:

Valid submissions will be evaluated and voted on by the Awards Selection Committee. Two finalists will be selected for the Service Management In Action award and the winner will be announced at FUSION 17.   

The two finalists will be expected to attend FUSION 17 where the overall recipient of this prestigious Award will be announced and awarded.  itSMF USA will provide each of the 2 Finalists with a FUSION 17 FREE Conference pass and a travel stipend of $1,000 to apply to the costs associated with your attendance at FUSION 17 in Orlando.

Please note that finalists could be invited to present their Service Management in Action initiatives via a short video presentation which may be aired during the event at the current or following year annual FUSION event, and/or to present on their project/initiative for an itSMF USA webinar or other virtual event.

All submissions will be acknowledged within three (3) business days.
